Main duties

  • Incumbent will be required to provide medically related transportation for community members focusing on the elderly and adolescents that are recognized with lack of transportation resources.
  • Incumbent's primary responsibilities are to provide home health visits to high risk elderly and adolescent clients in coordination with the Public Health Nursing and or Director of Nursing.
  • Provides education on healthy lifestyle habits to all clients.
  • Notify clients of scheduled medical appointments and follow-up from PHNs, Charge Nurse and referred providers appointments.

About the role

This position will provide quality outreach health care services and community-based health promotion and disease prevention services. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).

What you will do

  • Incumbent will be required to provide medically related transportation for community members focusing on the elderly and adolescents that are recognized with lack of transportation resources.
  • Incumbent's primary responsibilities are to provide home health visits to high risk elderly and adolescent clients in coordination with the Public Health Nursing and or Director of Nursing.
  • Provides education on healthy lifestyle habits to all clients.

  • This is a designated position covered by Public Law 101-630, requiring contact or control over Indian children.

  • Background Investigation Requirement: This position requires the successful completion of a federal background investigation as a condition of employment.
