Social Worker (Program Coordinator)

About the role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program.

You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.

Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

What you will do

• VA Careers - Social Work: VA Careers - Licensed Clinical Social Worker: The Dementia Care Coordinator is is a Social Work Program Coordinator whose primary responsibilities are to provide clinical evidence based services including but not limited to the following: Provide case management services to Veterans and their families throughout the continuum of care.
• Independently implement treatment modalities, provide counseling and/or supportive groups for Veterans and families.
• Provide consultation to other treatment team and staff members regarding psychosocial needs of Veterans and/or their families and the impact of the identified psychosocial problems on the Veteran's health care planning, compliance with treatment, and discharge planning.
• Serve on committees, work groups, and task forces at the facility as deemed appropriate by the supervisor and Chief of SWS.
• Responsible for furthering one's own professional growth through continuing education, for ensuring continuing education appropriate to the area of assignment Provides assistance with the orientation and learning of less experienced Social Workers.
• Provides clinical supervision to social worker's seeking independent license and/or accept asocial work trainee.
• Demonstrates knowledge and skill in the use of software applications for drafting documents and data management as well as other computer systems in use by the VHA.
• Maintains a level of productivity and quality consistent with the complexity of the assignment and consistent with Social Work standards, Long Term Care Institute standards, and facility standards.
• Consults with their supervisor as needed and with colleagues to develop knowledge and apply agency policies and practices.
• Documents social work interventions and activities in the Veteran's clinical record utilizing CPRS, in accordance with facility policy, and ensure appropriate hand off when transferring of patient care.
• Provides care coordination for Veteran's discharged from the geropsychiatric unit at 1 week and 1-month post discharge to help with adjustment to a new setting and answer questions/provide advice about potential or real behavioral concerns and help prevent the need for readmission.
• Contacts social services at institutional settings (i.e. ACHs) for Veteran's discharged from the geropsychiatric unit at 1 week to help with adjustment to a new setting and answerquestions/provide advice about potential or real behavioral concerns and help prevent the need for readmission.
• Prepares Veterans and their families for dealing with major neurocognitive disorders by providing training or consult services to SAVAHCS providers on the importance of and need for, advanced care planning for Veterans with, or at risk for, major neurocognitive disorders.
• Work Schedule: Monday-Friday, 7:30am - 4:00 pm EDRP Authorized: Former EDRP participants ineligible to apply for incentive.

• Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
• Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
