This position is eligible for the Education Debt Reduction Program EDRP Join the VA Black Hills Health Care System in Fort Meade, South Dakota, as a Community Living Center Registered Nurse and provide compassionate, resident-centered care to Veterans.

Coordinate care, administer treatments, support independence, collaborate across disciplines, and strengthen safety, quality, and continuity of care.

  • As part of an interdisciplinary team, you will combine hands-on nursing care with care coordination, resident and family engagement, and evidence-based practice to meet each Veteran's individual needs.
  • This position also offers opportunities to serve as a clinical resource, collaborate across services on complex care needs, contribute to performance improvement, and help maintain a safe, therapeutic environment for Veterans receiving continuing care.
  • Duties include, but are not limited to: Assess, plan, implement, and evaluate individualized nursing care for Community Living Center residents while coordinating services across the continuum of care.
  • Administer medications, treatments, and nursing procedures while promoting safe, effective, and resident-centered care.
  • Engage Veterans, families, and caregivers in care planning, incorporating their needs, preferences, values, and goals into treatment decisions.
  • Collaborate with interdisciplinary team members and other services to coordinate complex care needs and create a seamless experience for Veterans.
  • Serve as a clinical resource and liaison between nursing staff and the Nurse Manager to support communication and coordinated care delivery.
  • Conduct clinical and environmental rounds to evaluate care delivery, assess the therapeutic environment and safety, and identify opportunities for program improvement.
  • Apply evidence-based nursing practice, research, and current literature to strengthen care delivery and support quality and performance improvement initiatives.
  • Participate in the development and implementation of professional nursing standards and help ensure appropriate nursing coverage to support continuity of resident care.

  • We cannot guarantee a permanent day or night shift. - Night Shift Rotation: Staff assigned to day shifts will rotate onto night shifts for a two-week period based on seniority. - Rotating hours, weekends and holidays Telework: Not Available Virtual: This is not a virtual position.

Requirements

It is a requirement that all HCP to receive annual seasonal influenza vaccination or obtain an exemption for medical or religious reasons. Wearing a face mask is required when an exemption to the influenza vaccination has been granted. HCP in violation of this directive may face disciplinary action up to and including removal from federal service.

HCP are individuals who, during the influenza season, work in VHA locations or who come into contact with VA patients or other HCP as part of their duties. VHA locations include, but are not limited to, VA hospitals and associated clinics, community living centers (CLCs), community-based outpatient clinics (CBOCs), domiciliary units, Vet centers and VA-leased medical facilities.

HCP include all VA licensed and unlicensed, clinical and administrative, remote and onsite, paid and without compensation, full- and part-time employees, intermittent employees, fee basis employees, VA contractors, researchers, volunteers and health professions trainees (HPTs) who are expected to perform any or all of their work at these facilities.

HPTs may be paid or unpaid and include residents, interns, fellows and students. HCP also includes VHA personnel providing home-based care to Veterans and drivers and other personnel whose duties put them in contact with patients outside VA medical facilities.
