About this job
The Patient Access Hospital RN is responsible for responding to hospice referrals and for performing admissions in contracted facilities. Act as a partner in care with physicians, case managers and other facility staff. Educate patients and families about the hospice benefit and services. Establish eligibility and facilitate the coordination of discharge planning with hospice and hospital staff. Provide initial physical assessment, admission to hospice as required.
