About the Position: This position is located at the 27th Medical Group at Cannon AFB, NM. This is a Direct Hire Solicitation. Salary negotiation may be available for those candidates who are new to Federal service.

What you will do

• Perform difficult and complex assessments.
• Evaluate and analyze health care plan and resources.
• Communicate with treating facilities, both in direct and purchased care to coordinate patient care and transitions.
• Monitor, track and intervene as patients transition between levels of care.
• Develop and recommend strategies to improve healthcare access, reduce administrative barriers for beneficiaries, and enhance the efficiency of the MTF.
