UPMC is hiring a Discharge Plan Manager to join our Clinical Care Coordination team in Harrisburg! This is a full time, evening shift position with the final candidate working between the hours of 10a-8p, or 8a-8:30p. An every 5th weekend and rotating holiday commitment is required.
